Assists persons with mental illness to choose, find, and keep their own place to live, through continuous support services and through the development of resources for safe, comfortable and affordable housing. Provides linkage to housing subsidies and vocational services, assessment, continuing support with daily living and interpersonal skills, short-term crisis stabilization and permanency planning. On My Own is a collaborative program of Catholic Charities, Greater Trenton CMHC and the Association for the Advancement of Mental Health.
