SUPPORTIVE HOUSING

POSITION: HOUSING COUNSELOR / FT

Assists adults with a serious and persistent mental illness, who are enrolled in a GTBHC program, in finding and maintaining an apartment. Eligible individuals are homeless, at imminent risk of homelessness, or living in transitional housing programs. Services include identifying apartments, assistance in obtaining subsidies and ongoing support services related to landlord/neighbor relationships, employment, skill development, linkage to services, illness management and recovery services and other supports enabling consumers to maintain housing. Support is available 7 days per week, with 24 hour on call availability.

JOB REQUIREMENTS:

• Bachelor’s degree with two years’ relevant experience: or a combination of one or more years of college plus related work or life experience equaling four years or a high school diploma or equivalent plus four years of related work or life experience.
• Hours include some evenings and weekends.
• Rotating 24 hour on-call responsibilities.
• Use of personal vehicle may be necessary in carrying out job responsibilities.
• Valid driver’s license required.

 

JOB RESPONSIBILITIES:

• Function as part of an interdisciplinary team working with adults with a serious and persistent mental illness.
• Assist consumers in obtaining and moving into new apartments.
• Provide case management services including assistance with daily living skills, linkage to services, transportation, medication monitoring, etc.
• Conduct outreach visits to clients’ homes in the community.
• Provides crisis intervention and stabilization services as needed.
• On-call rotation required.
